2008-04-12: The agency responsible for Broward's foster children fired two employees on Wednesday after realizing they had previous criminal records and may have stolen from the agency.
ChildNet had discovered the men's previous convictions, including one for manslaughter, during background checks before they were hired four years ago, but the agency hired them anyway, said Peter Balitsaris, agency president.

2007-05-01: JP Morgan Chase has alerted thousands of its Chicago-area millionaire clients, as well as some of its own employees, that it ca not locate a computer tape containing their account information and Social Security numbers.
The tape, which was in a locked container, was being transported from a bank location to an off-site facility last month when it went astray, a JP Morgan spokesman said. It is not clear if the tape arrived at its destination or was lost along the way.

2007-06-19: A flash drive holding information on about 8,000 current and former Texas A&M University-Corpus Christi students was lost by a mathematics professor while on vacation in Madagascar, university officials said Friday.
The flash drive -- a thumb-sized memory data storage device -- may have contained Social Security numbers and other identifying information for students of all classifications and majors enrolled during the spring, summer and fall of 2006, said Marshall Collins, assistant vice president for marketing and communications at the university.

2007-02-19: Seton Healthcare Network is trying to recover the personal information of roughly 7,800 of its patients, after someone stole one of its laptops last week, a spokesperson for Seton said.
Seton says a thief stole the laptop from a leased office building in North Austin. The office is secured and visitors need a badge to enter.

Kaiser Permanente is in the process of notifying as many as 22,000 patients of a possible breach of their private medical information. There were no details provided about where or how the laptop was taken, but a Kaiser spokesman said it was likely a random and isolated crime of opportunity.

2007-02-07: Backup tapes with personal data on more than 135,000 Johns Hopkins patients, employees and retirees were lost by a contractor and are believed to have been destroyed, the hospital and university said Wednesday. The Johns Hopkins Hospital patient information on one missing tape included names and limited information such as sex and date of birth, but did not contain medical information, Social Security numbers, addresses or financial information, patients were told in an e-mail alert. However, information on eight missing Johns Hopkins University payroll tapes included Social Security numbers and, in some cases, bank account information for current and former employees. The tapes included information on retirees and students who have held campus jobs and involves employees from all university units except the Applied Physics Laboratory, the hospital and university said in a joint statement.

2007-03-27:Nottingham Primary Care Trust (NPCT) has admitted that "several laptops" have been stolen, one of which contains information on about 11,000 young children.The computer contained the names, addresses and dates of birth of children from the Newark, Mansfield and Ashfield areas, and was one of three stolen from an office at King's Mill Hospital in Sutton-in-Ashfield following a burglary last week.

2007-05-09: Marks & Spencer has confirmed that a laptop containing the personal details of
26,000 employees has been stolen.The laptop had been given to a printing company to allow it to write to the employees about pension changes.The data stored on the device included salaries, addresses, dates of birth, National Insurance details and phone numbers.
